REPORTING ERRORS IN THE NATIONAL PRACTITIONER DATABASE
If you notice an error regarding your facility, please let us know by filling out the form below. The National Provider List is a public document published regularly by Computer Sciences Corporation for The Centers for Medicare & Medicaid Services. As dialysis facilities expand, move, merge or are purchased, data changes need to be made. Most of these changes occur behind the scenes by your Network. This site serves as a valuable resource to make sure that changes to the database are accurate.
The Nephron Information Center does not change this database, but makes sure that the CMS data specialists are aware of errors, so that the master database can be appended
CMS asks that corrections be submitted from this page only for the information included in the National Provider Listing and only after the ESRD Network has been notified. Please include your dialysis unit provider number. CMS is not able to address other issues through these automatically-generated messages.
