REPORTING ERRORS

If you notice an error regarding your facility, please let your network know. The National Provider List is a public document published regularly by The Centers for Medicare & Medicaid Services. As dialysis facilities expand, move, merge or are purchased, data changes need to be made. Most of these changes occur behind the scenes by your Network. This site serves as a valuable resource to make sure that changes to the database are accurate. So, if you see something that needs to be corrected, let them know.

The Nephron Information Center does not change this database, but makes sure that the CMS data specialists are aware of errors, so that the master database can be appended
